Wisdom teeth are most commonly removed when their position causes pain, inflammation, crowding or other oral problems. A wisdom tooth that has not erupted correctly can also interfere with orthodontic treatment.
Thanks to modern oral surgery and anaesthetic techniques, wisdom tooth removal is now a safe and efficient procedure. Upright, partially or fully erupted wisdom teeth can often be removed within a few minutes. Even most cases requiring surgical exposure take no longer than approximately 20 minutes in experienced hands. Contrary to popular belief, not every wisdom tooth removal requires surgery. A correctly positioned, erupted wisdom tooth can often be removed quickly with a straightforward extraction.
Why do wisdom teeth often cause problems?
Partially erupted or unerupted wisdom teeth
If there is insufficient space or the tooth is incorrectly positioned, it may remain partly or completely beneath the gum or bone. Bacteria can accumulate under the gum covering a partially erupted tooth, causing inflammation, swelling, bad breath, painful swallowing and severe toothache. An impacted tooth may also press on neighbouring teeth or contribute to cyst formation and other surgical complications.
Impacted wisdom teeth
An angled or improperly erupted tooth can place pressure on the adjacent molar and, in severe cases, contribute to resorption of its root. Symptoms can include strong pressure-like pain, jaw tension, gum sensitivity and recurrent inflammation. Early diagnosis and appropriate removal can help protect neighbouring teeth and prevent later complications.
Difficult-to-clean tooth surfaces
Because of their position, wisdom teeth can be difficult to clean even with a small electric toothbrush head. Plaque, calculus and tooth decay are therefore more likely to develop. Decay may be detected late and can be difficult to treat because access is limited.
Complex root anatomy
Wisdom teeth often have curved, branching or difficult-to-access root canals. This can make root canal treatment technically challenging or unsuitable as a predictable long-term solution. Removal may therefore be recommended to prevent further pain, inflammation or complications.
Lack of space and crowding
Where a wisdom tooth cannot erupt properly or places pressure on the dental arch, it may contribute to tooth movement. An orthodontist and oral surgeon may recommend removal or regular monitoring to support the long-term stability of orthodontic results.
Chronic dental infection
A partially erupted wisdom tooth surrounded by inflamed tissue may become a persistent source of dental infection. Common symptoms include recurrent gum inflammation, swelling, bad breath and pain. Timely diagnosis and treatment are important to prevent recurrence and further complications.
What happens during the procedure?
Preparing for treatment
Appropriate imaging is always completed beforehand, using a panoramic X-ray and, where indicated, a CBCT scan. The specialist assesses the number and direction of the roots and identifies nearby anatomical structures such as blood vessels and nerve canals.
Choosing the appropriate anaesthesia
Most wisdom teeth can be removed under local anaesthesia. Sedation is also available at our clinic for anxious patients seeking a more relaxed treatment experience.

Gentle technique and expertise
Upright teeth can generally be removed like a conventional extraction, usually under local anaesthesia and without sutures. For a tooth lying within the bone, the area is carefully exposed and the tooth is usually divided into smaller sections before removal to minimise trauma to the jaw. The gum is then closed with fine sutures.
